Our family owned and operated nursing community offers skilled nursing, rehabilitation, long term care and assisted living under one roof. Colonial Poplin, our 50 bed Medicare and Medicaid certified skilled nursing/rehabilitation home, provides individualized services to meet diverse cognitive and physical abilities while Poplin Way Assisted Living gives residents the opportunity to live in their own apartments while receiving assistance when needed. At both Colonial Poplin and Poplin Way we believe in the person-centered approach to care, always treating our residents with dignity and respect, ensuring their total health needs are met, and they achieve their highest levels of functional independence.
