About ElderONE - Hudson PACE Center
ElderONE, an affiliate of Rochester Regional Health, is a Program of All-Inclusive Care for the Elder (PACE). Servicing Monroe, Ontario, and Wayne counties, ElderONE is an innovative alternative to long-term nursing home placement.
As the area's only PACE program, ElderONE represents the future of senior health care.
